About this tool
Add each medicine with the dose and the times of day, then print a weekly chart with boxes to tick for every day. It is a simple reminder for a patient or a caregiver.
How to use it
List every medicine, its dose and when it is taken. Print the chart and stick it where the medicines are kept. Tick a box each time a dose is taken so nobody misses or doubles a dose.
Important
This is only a reminder sheet. Follow your doctor's instructions and never change a dose because of a chart. Keep an updated list when the doctor changes a medicine.
