About this tool
Enter your modified adjusted gross income from two years ago (the year Medicare looks at), your filing status and how many people are on Medicare. The tool shows your tier, the monthly Part B and Part D amounts, the extra cost for the year and how far you are from the next tier.
How IRMAA works
The income-related monthly adjustment amount is an extra charge on Part B and Part D premiums for higher incomes. It is based on your tax return from two years earlier: 2024 income sets the 2026 premium. The tiers are cliffs: one dollar over a limit moves you to the next tier for the whole year.
Amounts
For 2026 the standard Part B premium is $202.90 a month, rising to $689.90 in the top tier, with Part D surcharges of up to $91.00. The 2025 figures are $185.00 to $628.90 and $0 to $85.80. The thresholds for 2026 start at $109,000 for single filers and $218,000 for married couples filing jointly.
Appeals
If your income fell because of a life-changing event such as retirement or the death of a spouse, you can ask Social Security to use a more recent year. Married filing separately has special thresholds that are not modeled.
Worked example
A single person with $150,000 of income two years earlier in 2026 is in tier 3: Part B is $405.80 a month and the Part D surcharge $37.50. The extra cost is $2,435 for Part B plus $450 for Part D, $2,885 a year, and $21,000 more income would move them to tier 4.
Tips for best results
Check every box before you rely on a result, because one wrong digit changes the whole answer. Use the exact figures from your payslip, bill, certificate or contract, and keep a note of the date you calculated. Rates, thresholds and rules change from time to time, so for tax, fees and legal matters treat the output as an estimate and confirm with the official source named on this page. You can run as many scenarios as you like; nothing is saved or sent, and refreshing the page clears the boxes.

Frequently asked questions
Which year's income is used?
The tax return from two years before, or one year before if it is not available.
Is Part D included?
Yes, the Part D surcharge is listed. It is paid with your plan premium, not the plan itself.
Can I lower it?
Appeal after a qualifying life event, or plan Roth conversions and gains in earlier years.
Does the surcharge count for both spouses?
Yes. Each person on Medicare pays it, so choose '2' for a couple.
